Nomination Form
The Stroke Foundation is
only able to accept nominations from within Australia. We regret that any nominees
currently living overseas will not be considered during the judging process.
Please note this nomination
is for the 2021 Stroke Foundation and Stroke Society of Australasia Stroke Care
Champion Award. Other categories of the Stroke Foundation Stroke Awards will
open later this year.
· Awards
are open to previous applicants who have not won.
Previous
winners cannot nominate for the Stroke Care Champion Award if they have been
awarded it previously.
Finalists will be notified at the end of start of
September. The winner will be notified in October.
The
Stroke Foundation and Stroke Society of Australasia’s decision is final. Please
note award finalists and winners may be featured in Stroke Foundation’s media
and publicity activities.
We
suggest you prepare your answers in a word document and copy into the
nomination form once you are ready.
