· Winners of each category will be announced in May 2021.
· The Stroke Foundation’s decision is final.
· Please note that award finalists and winners may be featured in Stroke Foundation’s media and publicity activities.
· We suggest you prepare your answers in a word document and copy into the nomination form once you are ready.
· Feel free to send additional material supporting your nomination to media@strokefoundation.org.au
